Client Forms

Client forms are required for all appointments scheduled through the MOMVS Pet Portal. These forms help us get to know you and your pet, understand your goals, and ensure safe, personalized care.

Forms are automatically emailed after scheduling and can be completed digitally. If you do not receive them, please check your spam folder or contact info@mcfaddinonthemove.com. PDF versions of the forms are also provided below for reference.

All clients must complete required forms prior to their initial appointment, including returning clients. Forms only need to be completed once.

Recheck appointments include a brief questionnaire to update your pet’s history, allowing more time during the visit for hands-on treatment and exercises.

Client Acknowledgement and Consent for Patient Restraint

Integrative and rehabilitation treatments often require patients to remain reasonably calm and safely restrained during evaluation and treatment. Client assistant, in a manner safe for the client and pet, may be required.
